COVID-19 Vaccine Registration

Pfizer (1st and 2nd doses) are available for anyone 12 and older. Anyone under 18 years of age must have a parent or guardian with them to be vaccinated. Moderna doses are for anyone 18 and older.
1st Booster Doses are available for Pfizer and Moderna. In order to be eligible, you must wait at least 5 months since a second dose of Pfizer or Moderna. Boosters for Pfizer are for 5 and older. Boosters for Moderna are 18 years or older.
2nd (Additional Booster) – for Pfizer and Moderna only. ADDITIONAL BOOSTER shot must be at least 4 months after receiving their initial booster. Other specifications for receiving this booster can be found here.
Under 5 Pediatric (Infant) COVID-19 vaccine is now approved for children aged 6 months – 4 years old. The Pfizer vaccine is given in 3 doses for 6-month-old – 4 years old with the initial doses 3 weeks apart and the third dose at least 8 weeks after the second dose. The Moderna vaccine is given in two doses for 6-month-old – 5 years old. The vaccine is administered as a two-dose series, 1 month apart. A third primary series dose may be administered at least 1 month after the second dose to individuals who are determined to have certain kinds of immunocompromise.
Pediatric COVID-19 vaccine is now approved for children aged 5-11 years old. The dose is 1/3 of the adult dose for each of the 2 doses given 21 days apart. Boosters are available 5 months after completing the initial two doses. You are encouraged to talk with your child’s pediatrician to determine what is best for your child.
Homebound individuals requesting 1st booster, Additional booster, or 3rd dose will be put on a list and will be seen as the availability of our nurses opens up. This may take some time, but remember you currently have some protection. We encourage those who are able to seek other means if they want their booster or 3rd dose earlier than we are able to provide it.

For First (1st) Dose:
- Please make sure you keep that appointment.
- Bring your confirmation e-mail/text with you.
- You will have to stay for approximately 15 minutes after the vaccine to be observed for possible side effects.
For Second (2nd) Dose:
- You need to bring your COVID-19 Vaccination Card with you.
- Return to where you originally got your first dose.
- You will have to go into the building for the vaccine.
- You will have to stay for approximately 15 minutes after the vaccine to be observed for possible side effects.
For an Additional Dose (Third Dose):
- A 3rd dose of COVID-19 mRNA vaccinations (Moderna and Pfizer) is currently recommended for those with compromised immune systems.
- A third dose of the Pfizer pediatric vaccine is available for children 5-11.

For First (1st) Booster Shot:
- A booster shot of the COVID-19 Pfizer vaccine is currently recommended for anyone over the age of 12 and for Moderna vaccine, anyone over the age of 18, and at least six months after completion of their initial series (two doses).
- For individuals who received the Johnson & Johnson (J&J) COVID-19 vaccine, booster shots are also recommended for those who are 18 and older and who were vaccinated two or more months ago.
For an Additional (2nd) Booster Shot for:
- Adults ages 50 years and older may choose to receive a second booster dose of an MRNA vaccine (Pfizer or Moderna) at least 4 months after their first booster dose.
- Adults ages 18-49 years who received Johnson & Johnson COVID-19 vaccine as both their primary series dose and initial booster dose may receive a second booster at least 4 months after their booster dose.
- Anyone 12 years and older who are moderately or severely immunocompromised may choose to receive a second booster dose of an mRNA COVID-19 vaccine (Pfizer or Moderna) at least 4 months after their booster dose.
